We are a private dental practice and we will bill all indemnity insurance plans.  We are a Delta Dental Premier provider and not contracted with other dental insurance companies.  As a courtesy to our patients, we will bill your insurance for services rendered and have payment issued to David E. Parker, DDS (Parker Dental Care). As a practice that is not contracted with your insurance company and considered out of network, payment may be issued directly to the patient from the insurance company and you will have a 20 day grace period to submit balance in full to Parker Dental Care.

We do our best to give you an accurate estimate although due to different policies and usual and customary rates of insurance companies, we are unable to guarantee any estimate.  If for any reason your insurance company fails to pay within a reasonable period of time, we will collect from you any insurance estimates not paid. Please inform the front desk of any changes to your insurance coverage.
